One could look at this question as guaranteed suffering (Pain) vs. possible-yet-not-certain suffering (Fear).
Pain is, in and of itself, agony. Whether it's a searing fire engulfing your hand or your psyche slamming itself against your skull, pain is by definition suffering.
There is no pain without suffering, yet with the right kind of acutely grounded approach, there can be fear without suffering. Fear is a mental and emotional response. It is entirely internal, and although it often leads directly to pain, it does not have to.
Attachment to fear is a consensual partnership, where one often subconsciously relies on their fears to provide them a sense of self and identity. Observation of fear, however, can help one realize how truly separate it is from them, and how their fears are not at all definitive of who they are.
Once that detachment from fear is accomplished, it becomes much easier to recognize, observe, and accept fear without any suffering at all. Pain becomes optional and delightfully unnecessary :)
